A Lifeline for Long-Term Treatment

Diabetes. High blood pressure. Seizures. Heart disease. In rural Haiti, chronic conditions can become life-threatening when care and medication are out of reach. FOTCOH’s Vital Health Clinics provide returning patients with treatment, follow-up care, and medicine they can rely on.

When Consistent Care is Difficult to Access

For many patients in rural Haiti, conditions like diabetes, hypertension, seizure disorders, heart disease, and other long-term illnesses are difficult to manage because reliable care and essential medications are often out of reach.

Without ongoing treatment, manageable conditions can quickly become dangerous. Patients may travel long distances, sometimes over multiple days, to reach care they can trust and medications they may not be able to access anywhere else.

Mario’s Story

“The medicine was out of reach. So was hope. I had given up. The medicine was too expensive and too hard to find. But at FOTCOH, I got the care I needed to survive.”

Mario had been living with diabetes and high blood pressure, but without reliable access to medication, his condition became critical. The treatment he needed was too expensive, too difficult to find, and eventually impossible to continue.

When Mario came to FOTCOH, the Haitian medical team stabilized him, restarted his treatment plan, and connected him with medication and follow-up care he could rely on.

Today, Mario has more than medicine. He has stability, hope, and a path forward.

Our Goal

Providing Care Patients Can Return To

FOTCOH’s Vital Health Clinics are designed for continuity, not one-time treatment. Held during the first month of each quarter, these intensive multi-day clinics provide evaluations, treatment plans, follow-up care, and access to essential medication through FOTCOH’s pharmacy.

Patients managing chronic and complex conditions are prioritized, helping them stay connected to the long-term care that can improve health, stability, and quality of life over time.
